“Genogram Analysis: Identifying Risk Factors and Developing Nursing Interventions for Health Promotion”

Professionals in many fields that study and work with people use genograms. They provide a diagram of a person’s family and social network to identify patterns and trends across multiple generations. Genograms can be clinically beneficial to integrate a patient’s family information into the health assessment for improved patient care. In this discussion board, you will first complete a genogram on your selected patient (the same patient that you used for the Complete Health History Assessment in Modules 1, 2, and 3). **Patient chosen is a friend.**
Go to one of the following websites to create either a web-based or pdf printable version of your selected patient’s family genogram:
The Centers for Disease Control and Prevention web-based genogram tool.
U.S. Department of Health and Human Services web-based and printable versions of the “My Family Health Portrait Tool”
After completing the genogram, evaluate how its data supports or enhances the body of knowledge gathered in the complete health history – what new information was revealed that will direct health promotion or disease prevention for your patient?
Directions
Complete the genogram on your selected patient using one of the tools above.
Compose a short essay (250-300 words) to address the following areas:
Identify one real or potential risk factor that was revealed (or reinforced) by the genogram.
Develop nursing interventions for health promotion related to this risk factor.
Cite at least two scholarly articles from the nursing literature that support your content. Additional information can be found on SUNY Empire State University’s Library website.
Compare your information with peers in a discussion forum.
Follow all guidelines of the discussion rubric for this activity.
You do not need to submit the genogram.
This activity will be graded using the discussion rubric.
Discussion Participation Tips
Your initial post should be a reflective response that answers all questions, is adequately detailed, and cites outside sources when necessary to support your post. Typically, a post should be at least 250 words to allow you to answer and ask follow-up questions, provide supplemental information, and to share views and experiences that draw us into an in-depth and meaningful conversation. Your first response should be posted by midweek and then you should return to respond to at least two other students during the remaining week(s) of the module. Be sure to use APA format to cite sources when putting forth the ideas, opinions, and facts of others.
