Assignment Summary
Purpose: Identify a problem or issue in your practice environment. Then identify a nursing theory that can provide a perspective and describe how you would apply the theory in practice to address the problem.
Practice Problem: Obesity related comorbidities are prevalent in primary practice patient populations. Heart disease, type 2 diabetes, high cholesterol, sleep apnea, GERD, amongst other conditions, are health conditions patients present with continually for management. Many of these conditions are deemed untreatable, if obesity is not resolved. Encouraging patients to adopt healthy behaviors required to manage obesity and prevent or manage illness is a fundamental aspect of primary care but can be challenging.
Nursing Theory: Pender’s Health Promotion Model
Application: This model focuses on individual characteristics and experiences, behavior-specific cognitions, and behavioral outcomes. Nurse Practitioners can use this model to develop personalized interventions that motivate patients to engage in health-promoting behaviors, such as regular exercise, healthy eating, and smoking cessation.
Specific proposed change:
Specific focus for the patient population would be recommendation of AOMs (Anti-Obesity medications) like GLP-1 injections such as semaglutide and tirzepatide, to help manage obesity coupled with motivating patients to engage in health promoting behaviors to treat obesity related conditions which are impacting the patient’s quality of life.

Clearly describe the selected problem as it relates to nursing practice. Substantiate the problem with one piece of internal evidence from current studies in the primary practice environment.
Provide a comprehensive and concise summary of current evidence pertinent to the identified problem. Summary should be well organized. Support the content with references to evidence from a minimum of 10 scholarly, peer reviewed sources.
Clearly describe the nursing theory or model that can be used in practice that addressed the problem. Identifies the major concepts in the theory. Provide an original citation for the theory.
Clearly describes a specific proposed change in nursing practice based on the analysis of the selected identified practice issue and theory.
Clearly identify and describe the changes proposed in a minimum of 3 SMART goals.
