Choosing a PICOT Question for Vulnerable Populations in Community Health
Capstone Part I: PICOT Assignment (NURS 7000) – Fall 2026
Advanced practice nursing students completing Capstone Part I must design a PICOT question addressing a non-saturated health issue affecting a vulnerable population, supported by national and local data, five peer-reviewed sources, and a nursing theoretical framework, to propose a feasible evidence-based intervention.
This assignment requires you to identify a health problem at your workplace or in your community that avoids the saturated topics listed below. You will write a five-page APA 7 paper proposing an evidence-based intervention for a vulnerable population. Your paper must include a detailed PICOT question, social determinants of health analysis, five peer-reviewed sources from the last five years, a nursing theory framework, and a realistic implementation timeline. The rubric weights problem significance, population description, literature synthesis, theoretical grounding, and writing mechanics equally at 20 points each, with sources and evidence at 10 points.

Choosing a fresh topic signals scholarly originality to faculty reviewers and mirrors the competency expectations for DNP-level practice inquiry. A well-scoped problem tied to measurable outcomes will serve you across Parts II and III of the Capstone sequence.
PICOT Assignment Instructions
Title Page
Use appropriate title for faculty. Example: Dr. Carmen Lazo, DNP, MSN, APRN-BC, APN.
Introduction to Your Proposed Problem
Significance of the Practice Problem: Discuss the identified practice problem. Introduce the topic of the paper (Abstract, Introduction). Describe the health problem.
Using data and statistics, support your claim that your selected issue is a problem. What is the cost (national and local) and budget (if any) for your proposal? What is the cost nationally and locally?
Include your purpose statement and what specifically you will address in your proposed program. Be sure your proposed outcome is realistic and measurable. Align with the semester hours.
National data alone cannot capture the neighborhood-level variation that shapes your population’s experience. Pair national statistics with county or municipal health department data to demonstrate local relevance.
PICOT
Should be discussed in detail. This section should include your PICOT question but also should provide thorough descriptions of your population, intervention, comparison intervention, outcome, and timing.
The word PICOT is a mnemonic derived from the elements of a clinical research question: patient, intervention, comparison, outcome, and time. The PICOT process begins with a case scenario, and the question is phrased to elicit an answer. Present the PICOT in APA Essay format. Do not use bullet points.
- P – Patient/Problem
- I – Intervention
- C – Comparison
- O – Outcome
- T – Timeframe
Vulnerable Population and Setting
Be specific. Discuss the impact of social determinants on health for your selected population.
What are the risk factors that make this a vulnerable population? Why is change important for this population? What are the current statistics and cost? Use national and local data on your population’s setting. Where are they existing? Example: Miami-Dade.
Use evidence to support the risk factors you have identified.
Research and Literature Review
Research should be from scholarly journals of evidence-based interventions that address the problem. Do not use websites as substitutes. At least 5 scholarly articles and if applicable scholarly websites.
Peer-reviewed research articles not older than five years (all articles must be used in your paper).
Synthesize your sources thematically rather than summarizing each article one by one. Group findings by intervention type, population characteristic, or outcome domain to show analytical depth.
Proposal
Propose addressing the selected health problem using an evidence-based intervention in your literature search to address the problem in the selected population/setting.
Include a thorough discussion of the specifics of this intervention which include resources necessary, those involved, and feasibility for a nurse in an advanced role.
Be sure to include a timeline for the intervention proposed.
Theoretical Framework / Nursing Theory
This section should include the theoretical framework which supports your project. Describe a theory or model to serve as the foundation for your project. How will this theory be incorporated into your Capstone? Discuss in detail.
Overall, you should be able to introduce the problem and why this is significant to the healthcare system. For example: What impact does it have on the patient, community, cost of care, quality of life, readmissions and so forth?
- Use appropriate APA 7th Ed. format. Write PICOT in ESSAY format.
- Scholarly, peer-reviewed, and research articles cited should be within the last five years.
- This section should be minimum 5 pages long (not including the title and reference page).
- Use proper in-text citations with a properly formatted reference list.
- All papers must be written in the 3rd person.
Rubric Grading Part I
| Criterion | Points | Requirements for Full Credit |
|---|---|---|
| PICOT and Proposed Health Problem | 20 | Thoroughly describe the proposed health problem and clearly explain what change is being proposed. Support the significance of the problem with current data, statistics, and scholarly literature. Clearly connect the problem to the PICOT question. |
| Description of the Population | 20 | Provide a detailed description of the target population, including relevant age range, demographic characteristics, setting, health condition, and socioeconomic or clinical factors. Identify important risk factors affecting the population and support them with current scholarly evidence. |
| Depth of Discussion and Literature Review | 20 | Provide an in-depth discussion and elaboration throughout the entire paper. Include a literature review from scholarly journals describing evidence-based interventions that address the identified health problem. | TOTAL | 100/100 | All Outstanding criteria must be fully addressed. |
Sample Answer and Example Essay Content
Food Insecurity as a Practice Problem
Food insecurity affects roughly 13.5% of U.S. households, a figure that climbs to nearly 30% among families with young children (Children’s HealthWatch, 2024). The health consequences include higher rates of chronic disease, poor mental health, and increased healthcare utilization. Annual healthcare and educational costs attributable to food insecurity reach an estimated $237 billion. Among populations experiencing food insecurity, Drydakis (2023) found that physical health deteriorations correlate with older age, female gender, immigration status, disability, and long-term health conditions. These intersecting factors create layered vulnerability that a single intervention cannot easily resolve.
Nurses working in community health settings encounter food insecurity frequently but often lack a structured screening and referral protocol. An advanced practice nurse-led intervention could implement standardized screening during primary care visits, followed by warm handoffs to community food resources and nutrition education. Such a program aligns with the Health Promotion Model, which focuses on individual behavior characteristics, behavior-specific perceptions, and anticipated behavior outcomes (Bulcao Santi et al., 2023). The model’s emphasis on psychological and socioeconomic factors makes it well suited for addressing food insecurity as a health behavior challenge rather than a purely economic one.
Vulnerable Population Focus
The target population for this project consists of low-income adults aged 18 to 65 receiving care at a federally qualified health center in Miami-Dade County. Social determinants of health such as economic instability, limited education access, and neighborhood deprivation disproportionately affect this group. Walker et al. (2024) demonstrated that neighborhood violence and educational attainment independently associate with adverse health outcomes, a pattern that mirrors the chronic stress pathways linking food insecurity to poor health. The prevalence of food insecurity in Miami-Dade exceeds the national average, making local intervention both urgent and feasible.
Risk factors include unemployment, housing instability, single-parent household status, and limited English proficiency. Change is important because food insecurity predicts emergency department overutilization, lost productivity, and preventable hospitalizations. A nurse-led screening and referral program could reduce these downstream costs while improving patient-reported outcomes within six months. The proposal requires minimal material resources: a validated screening tool, staff training, and a maintained community resource directory. Feasibility for an advanced practice nurse is high because the intervention integrates into existing clinic workflows without requiring new staffing or physical infrastructure.
Research, Writing, Citation & Referencing Guide
This section offers practical guidance for strengthening your Capstone Part I paper. Use the answer-first structure shown above as a model. Open each major section with a direct statement of what the reader will learn, then support that statement with evidence. Cite named frameworks such as the Health Promotion Model or the Social Determinants of Health framework. Reference professional standards from the CDC, NIH, or WHO when discussing population data. Include one short FAQ that students commonly ask about PICOT assignments, followed by an evidence-based answer. Explain why the topic matters in practice by connecting it to workplace applications. Use semantic keyword variation rather than repeating identical phrases. Suggest descriptive internal links if you publish your work online. Prioritize verifiable facts and recent scholarly sources to strengthen E-E-A-T signals.
Why This Matters in Practice
Food insecurity screening in primary care settings has demonstrated measurable improvements in referral completion and patient-reported food access. A nurse-led program can bridge the gap between clinical care and community resources. The skills you develop in this assignment, including problem identification, evidence synthesis, and theory application, transfer directly to DNP-level practice inquiry and quality improvement work.
Common Question Students Ask
How do I choose a PICOT question that satisfies the rubric’s originality requirement?
Select a health issue you observe regularly in your practice setting but that does not appear on the prohibited topics list. Ground the problem in local data whenever possible. Then verify that at least five peer-reviewed sources from the last five years address an intervention for that problem. If the literature is thin, narrow your population or setting to create a more focused question.

Next Assessment: Week 4 Discussion Board
NURS 7000 – Week 4 Discussion: Literature Search Strategy and Source Evaluation
For this discussion, you will conduct a database search for evidence supporting your PICOT question. Post a 300- to 400-word summary of your search strategy, including the databases you used (CINAHL, PubMed, or Cochrane Library), the Boolean operators and keywords you combined, and the inclusion and exclusion criteria you applied. Then, select two peer-reviewed articles from your results and evaluate each using the Johns Hopkins Evidence-Based Practice appraisal tools. Explain why each source is appropriate for your Capstone Part I paper and how it addresses your population, intervention, or outcome of interest. Respond to at least two peers with constructive feedback on their search strategies and source selections.
